Position Description:
A Home Health Aide (HHA)/ Caregiver is a non-licensed member of the home health care team who assists the client with the tasks of daily living as outlined in a written care plan that is established by a Care Manager/Clinician. HHA/Caregivers are supervised by the Client Services Coordinator with oversight of the Director/Administrator at this agency.
This particular position will require mainly work with hospice patients.
Qualifications/ Requirements:
1. A Home Health Aide/Caregiver must have related work experience, which is verifiable.through references, and must complete the agency’s employment process.
2. High School diploma or equivalent.
3. CNA certification preferred.
4. Must be at least 18 years of age.
5. Must be physically able to perform the duties of the position.
6. Must exhibit mature responsible behavior, and understand the need for patient confidentiality.
7. Must be able to follow direction and accurately report to the Care Manager/clinician a changes in client's condition.
8. Must have available reliable transportation to and from assignments.
