Location: Fayette County, PA

Status: Per Diem

Position Description: The Licensed Practical Nurse under the supervision of the Nurse Manager and a registered nurse , will administer skilled nursing services within their scope of the licensure to clients in a home care setting. Provides services in accordance with physician orders, established plan of treatment and the organizations policies and procedures. Will educate the patient/caregiver in the management of their illness and promote the home care support systems to decrease re-hospitalization and utilization of hospital emergency services.

Qualifications / Requirements: Licensed LPN with current state license. EXPERIENCE: One year prior nursing experience required; Hospice/Home Care experience preferred. Ability to work independently and within a multidisciplinary team. Valid drivers license necessary.
